Who needs a DOT medical card?

You need a DOT medical card if you operate a commercial motor vehicle (CMV) that:

Weighs over 10,000 lbs
Transports hazardous materials
Carries more than 8 passengers (for compensation)
Carries more than 15 passengers (not for compensation)

How long is a DOT medical card valid?

Typically valid for up to 24 months
Some drivers may receive shorter certifications (3, 6, or 12 months) depending on medical conditions like high blood pressure or diabetes

What should I bring to my appointment?

Bring the following:

Valid driver’s license
Glasses or contacts (if you use them)
Hearing aids (if applicable)

How long does the exam take?

Most exams take 15–30 minutes, depending on your medical history and any additional documentation needed.

What does the DOT physical include?

The exam includes:

Urinalysis (not a drug test)

Is this a drug test?

No. The urinalysis checks for basic health indicators like protein, blood sugar, and kidney function—not drugs.

What can cause me to fail a DOT exam?

What happens if I don’t pass?

Yes—if it’s controlled. Drivers with elevated blood pressure may receive a shorter certification and need regular monitoring.

Do I need a DOT exam if I’m self-employed?

Yes. Owner-operators must meet the same medical requirements as company drivers.

Can I use my own doctor?

Only if your doctor is listed on the FMCSA National Registry of Certified Medical Examiners.

How much does a DOT physical cost?

Costs vary by provider, but our exams $100. An additional $50 is charged if you have to come back to our office to complete certification, so remember to bring everything needed and required.

Do I need an appointment?

Yes scheduling ahead is the best way to get in and out fast. Walk-ins are discouraged with respect to your time and ours.

What if I have sleep apnea?

You can still qualify if:

Your condition is being treated
You are compliant with CPAP therapy
You provide usage reports if requested

What if I take prescription medications?

Most medications are allowed, but some may require additional documentation. Bring a list of all medications and prescribing doctors.

How do I prepare for my exam?

Get a good night’s sleep
Take your medications as prescribed
Avoid caffeine or nicotine right before your exam (can raise blood pressure)

Can I renew my DOT medical card early?

Yes. It’s recommended to renew before your current card expires to avoid any lapse in certification.
